a Pupil/Student application of the Basic Education Department (BED) must meet the following requirements for Admission

1. Filled-up Student's Application Form
2. Report Card with LRN (Learners Reference Number)
3. Original Copy of Good Moral Certificate
4. Clear Photocopy: PSA Birth Certificate
5. Four(4) copies of 2x2 recent ID picture (white background)
6. One (1) long brown envelope

1. Certificate of junior High School Completion
2. ESC Certificate Letter from previous school (ESC grantees)
